2 Pet Information

Please share why this appointment is needed. Examples: heart failure, seizures, kidney failure, not eating/drinking, cancer, arthritis, quality-of-life decline, etc.

3 Visit Preparation

Help us arrive prepared so we can give your pet the best possible care.

Examples: pneumonia, open wounds, skin infections, fly/maggot involvement, contagious illness, etc.

4 Service Request

Primary clinic notification: If your pet has been seen within the last 12 months, we will notify your primary clinic that their patient was humanely euthanized. If you choose private cremation, ashes return options (pickup, shipping, or delivery to your primary clinic) are listed in the aftercare section below.

5 Aftercare Options

Starting costs include euthanasia and the aftercare option selected. Final pricing is confirmed when we follow up.

Default is pickup at West Coast Memorial in Colbert. You may also choose shipping to your home or delivery to your primary vet clinic.

A standard urn is included with private cremation. You can confirm or change your selection and add engraving through the pet portal after your appointment. Specialty urns and memorial items are available at additional cost through West Coast.

6 Appointment Preferences

If booked in advance, a narrower arrival time can often be arranged. Appointments typically last between 15 minutes and 2 hours.

Please share anything that may help us make your pet's experience as peaceful and comfortable as possible. Optional — you don't need to cover every prompt. Examples: current condition, mobility, anxiety/handling concerns, medications already given, other pets or children present, who will be there, or anything else important.

7 Signature & Agreement

Your appointment is not confirmed until we follow up with you. Once confirmed, this signed form will serve as your consent document.

Payment: We accept cash, check (made out to Vet Valet), all major credit cards, Venmo, and PayPal. Payment details will be provided when we follow up to confirm your appointment.
The card processing company we use offers a 6-week payment plan option with a variable interest rate depending on approval. Please let us know if you would like to use this option.
